Agenda Item #3-Warrants
Vote to consider the approval of the following warrants:
• Warrant #34-Refunds in the amount of $245,207.43
• Warrant #35-August Retiree Benefits in the amount of $5,813,856.36
• Warrant #36-Expenses in the amount of $ 39,537.39

Agenda Item #4-Refunds
Vote to consider the ten (10) requests for refunds. The names have been submitted to their particular Department, the Law Department and the Massachusetts Department of Revenue.

Agenda Item #5-Redeposit of Refunds and Makeup of Contributions
1. Vote to consider the redeposit of a refund for Franzuas Madera. Refund taken in 2013. Rehired, July 20, 2015 as Principal Clerk for Commerce High School.

2. Vote to consider the redeposit of a refund for Scott Campion. Refund taken in 2014. Rehired, August 10, 2015 as a Firefighter.

3. Vote to consider the makeup of contributions for Aweys Ali. Purchasing part time prorated (11 months) employment as a School Custodian.

4. Vote to consider the makeup of contributions for Martin Germain. Police cadet time of 3 years and 3 weeks.

Agenda Item #6-Involuntary Ordinary Disability-Firefighter, William Sepulveda
Vote to consider the involuntary ordinary disability application submitted by Fire Commissioner, Joseph Conant. Mr. Sepulveda has also filed his own application for accidental disability.

Agenda Item#7-Involuntary Accidental/Ordinary Disability-Police Officer, Ricardo Ortiz
Vote to consider the involuntary accidental/ordinary disability application submitted by Police Commissioner, John R. Barbieri. Mr. Ortiz has also filed his own application for accidental disability.

Agenda Item #8-Survivor Benefits-Mrs. Barbara Ferris
Vote to consider section 101 benefits to the surviving spouse of, Police officer Stanley Ferris who retired in 1982 and passed away in 2015. Mrs. Ferris has indicated that she will attend and would like to address the Board.

Agenda Item #9-Survivor Benefits-Mrs. Judith Roberts/Doty
Vote to consider section 9 benefits to the surviving spouse of, Police Officer, Milton Doty who retired in 1997 and passed away in 2015.

Agenda Item #10-Outstanding 91A compliance
Vote to consider the termination of disability benefits to the following Accidental Disability Retirees that have not reported their 91A earnings to the P.E.R.A.C.: Retirees: James Kervick and Michael McClaflin.
