Vote to consider the payment of the following warrants:
# 36 Expenses in the amount of $3,000.00
# 37 September Retiree Payments in the amount of $5,614,677.56 including redemption of
$4,750,000.00 from the PRIT Fund
# 38 Refunds in the amount of $95,170.13
# 39 Expenses in the amount of $91,203.53

Agenda Item #4-Refunds
Vote to consider the twenty-one (21) requests for refunds. The names have been submitted to their particular Department, to the Law Department and to the Massachusetts Department of Revenue.

Agenda Item #5- Make-up of Contributions
Vote to consider the re-deposits of refunds for:
1.Anthony Restivo for the first 6 months of employment with the DPW

Agenda Item #6-Acceptance of Liability
Vote to consider the request to accept creditable service for Lori Stoddard as a Tutor (temporary position) for the Springfield School Department.

Agenda Item #7-Under $5,000 Rule, Section 5 of Chapter 21 of the Acts of 2009
Vote to consider the request from Marie Hurley, Crossing Guard for the School Dept. Ms. Hurley’s creditable service was adjusted down, pursuant to Section 5.

Agenda Item #8-Accidental Disability-David Starr/Police Department
Vote to consider the approval for accidental disability benefits with the evidence of the Involuntary application submitted by the Police Commissioner along with the medical panel reports for Officer David Starr of the Springfield Police Department. Represented by Attorney Kevin Coyle

Agenda Item #9-Accidental Disability-Michael Leblanc/Springfield Housing Authority
Vote to consider the approval for accidental disability benefits with the evidence of the Involuntary application, member’s application along with the medical panel reports for Michael Leblanc, Working Foreman. Represented by Attorney Charles Casartello, Jr.
