2. Dorothy Cook, Paraprofessional, School Department, with 10 years and 9 months as of 6/22/16

4. John Gigliotti, Foreman III, Water/Sewer Comm. with 28 years and 7 months as of 7/13/16

Vote to consider the approval of:
Warrant #16 Expenses in the amount of $21,140.36
Warrant #17 Refunds in the amount of $370,839.31
Warrant #18 Expenses in the amount of $107,685.29
Warrant #19 May Retiree allowances in the amount of $5,934,687.17 and redemption
from the PRIT fund in the amount of $5,425,000.00
Warrant #20 Expenses in the amount of $23,422.07

Agenda Item #5-Refunds
Vote to consider the thirty-five (35) requests for refunds. The names have been submitted to their particular Department, the Law Department and the Massachusetts Department of Revenue.

Agenda Item #6-Redeposit of Refunds and Makeup of Contributions
Vote to consider the redeposit of refunds to:
1. Rosemary Coughlin, Paraprofessional, School Department, for the refund from Springfield in 2007.
2. Tasheena Davis, Paralegal, Law Department, for the refund from Springfield in 2013.
3. Pamela Sergienko, Health Assistant, School Department, for the refund from Hampden County in 1990.

Vote to consider the makeup of contributions to:
1. Tasheena Davis, Paralegal at Law Dept, for12 months of employment at Law Dept., Law Clerk.
2. Mary DeVivo, Inspectional Service Clerk at Buildings Department, for 7 months of employment at School Department, cafeteria worker.
3. Lisa Lipshires, Librarian at Library Dept., for 1 year 1 month of employment at Springfield Library, as a part time Librarian.

Agenda Item #7- Accidental Disability-Linda Marciniec/School Department
Vote to consider the responses of the medical panel regarding Ms. Marciniec’s application for accidental disability benefits.

Agenda Item #8- Accidental Disability- Luis Astacio/DPW
Vote to consider the responses of the medical panel regarding Mr. Astacio’s application for accidental disability benefits. Tabled from April 12, 2016 meeting.

Agenda Item #9- Involuntary Accidental Disability-Brian Goggin/Fire Department
Vote to consider the acceptance of the involuntary disability application submitted by Fire Commissioner Joseph Conant, and request that PERAC convene a medical panel.

Agenda Item #10- Accidental Disability-Matthew Cincotta/Fire Department
Vote to consider the responses of the medical panel regarding Mr. Cincotta’s application for accidental disability benefits.

Agenda Item #11- Benefits under Section 101
Vote to consider survivor benefits to the beneficiary of Frank Winn III of the Fire Department-Deceased.
