MINUTES OF THE SPRINGFIELD RETIREMENT BOARD
HELD ON, JANUARY 7, 2015 AT 5:00 P.M.

PRESENT: Robert Moynihan, Haskell Kennedy, Philip Mantoni, Patrick Burns, Thomas Scanlon, Attorney Alfredo ViVenzio, Executive Director, Anne Leduc, Miguel Rodriguez,
Juan Ocasio, Attorney Kevin Coyle, Joanne Koetsch and Attorney Charles Casartello

Agenda Item #1--Minutes
Motion made by Kennedy, seconded by Mantoni to approve the minutes of the meetings held on, December 3, and December 31, 2014. Voted unanimously

Agenda Item #2-Superannuations
Motion made by Moynihan, seconded by Kennedy to accept the following Superannuation applications submitted by: Robert Barnard, Assistant Solid Waste Mgr. with 36 years and 2 months as of 2/3/15, Susan Lambert, School LPN with 16 years and 9 months as of 12/24/14, Douglas Rollins, Firefighter with 29 years and 10 months as of 2/3/15, Bruce Leiter, W & S Comm. Contract Admin with 25 years and 2 months as of 4/4/15 and Steven W. Flaherty, Firefighter with 26 years and 3 months as of 1/8/15. Voted unanimously

Agenda Item #3-Warrants
Motion made by Moynihan, seconded by Kennedy to approve the payment of Warrant #1 for expenses in the amount of $21,753.57. Voted unanimously

Agenda Item #4-Refunds
Motion made by Moynihan, seconded by Mantoni to Vote to approve the five (5) requests for refunds. Voted unanimously

Agenda Item #5-Redeposit of Refund and Makeup of Contributions
Motion made by Kennedy, seconded by Moynihan to approve the re-deposit of refunds and makeup for: Dorothy Flebotte for the first 6 months employed with S.H.A, Patricia Ryan for a redeposit of the refund in 2008, Martin Kelleher for a redeposit of the refund in 1986, Christine Tyburski for the first 6 months employed with W & S and Adalberto Reyes for a redeposit of the refund in 2006. Voted unanimously

Agenda Item #6-Involuntary Accidental Disability-Miguel Rodriguez/Fire Department
Motion made by Moynihan, seconded by Kennedy to accept the involuntary accidental disability application, submitted by Fire Commissioner, Joseph Conant, along with the voluntary Member’s application and request P.E.R.A.C. to set up a medical panel. Voted unanimously

Agenda Item #7-Accidental Disability-Peter Vera/W&S Commission
Motion made by Moynihan, seconded by Kennedy to accept the responses of the medical panel along with the application for accidental disability benefits and approve benefits. Voted unanimously

Agenda Item #8-Accidental Disability-Holly Clements/Fire Department
The member nor Attorney Sullivan were in attendance. The Chairman asked for a motion on the accidental disability application submitted by Holly Clements. Motion made by Philip Mantoni to deny accidental disability benefits, seconded by Haskell Kennedy. Motion passed by a majority of the Board. The Chairman asked for a motion on the Involuntary Accidental or Ordinary disability application submitted by Fire Commissioner, Joseph Conant. Motion made by Kennedy, seconded by Moynihan to approve Ordinary Disability Benefits. Voted unanimously

Agenda Item #9-Involuntary Accidental Disability-Juan Ocasio/Police Department
Motion made by Kennedy, seconded by Moynihan to accept the responses of the medical panel
along with the involuntary application and approve accidental disability benefits. Voted unanimously

Agenda Item #10-Involuntary Accidental Disability-Joanne Koetsch/School Department
Motion made by Moynihan, seconded by Kennedy to deny the Involuntary application based on the lack of information and the wrong job description. Voted unanimously. Attorney Casartello informed the Board that Joanne Koetsch was refiling a new accidental application.

Agenda Item #11-Accidental Disability-William Smith/School Department
Motion made by Moynihan, seconded by Kennedy to accept the responses of the medical panel along with the application and approve accidental disability benefits. Voted unanimously
