Agenda Item #1-Minutes
Vote to consider approving the minutes of the meetings held on July 12, 2016 and July 28, 2016.

Agenda Item #2-Superannuations
Vote to consider the acceptance of the following Superannuations:
1. Brendan Baker Jr., Firefighter, Fire Department, with 26 years and 5 months as of 9/19/16
2. Michael Downey, Water Resources Manager, Water & Sewer Commission with 34 years and 6 months as of 10/1/16
3. Timothy Duquette, Supervisor of Attendance-School Dept. with 25 years and 7 months as of 8/13/16
4. Michael P. Foley, Senior Custodian, School Department, with 24 years and 9 months as of 7/14/16
5. Ann Marie Griffin, Para Professional, School Department, with 16 years and 3 months as of 8/5/16
6. William Riordan, Senior Custodian, School Department, with 35 years as of 7/27/16
7. Roberto Robles, Police Officer, Police Department, with 17 years as of 8/8/16

Agenda Item #3-MGL Chapter 32 Hearing / Superannuation
Vote to consider the acceptance of Francis Audet’s Superannuation Application, Warehouse Foreman, Facilities

Agenda Item #4-Warrants
Vote to consider and concur with the approval of:
Warrant #25 Refunds in the amount of $330,075.09
Warrant #26 Expenses in the amount of $48,701.05
Warrant #27 Expenses in the amount of $4,888.85
Warrant #28 July Retiree allowances in the amount of $6,067,084.45

Agenda Item #5-Refunds
Vote to consider the nineteen (19) requests for refunds. The names have been submitted to their particular Department, the Law Department and the Massachusetts Department of Revenue.

Agenda Item #6-Redeposit of Refunds and Makeup of Contributions
Vote to consider the redeposit of refunds to:
1. Sian Simpson, Police Academy Officer, Police Department, for the refund from Springfield in 2015.

Vote to consider the makeup of contributions to:
1. Karen Cliché, Principal Clerk Typist, School Department, purchasing 4.5 months of employment at the school department as a part time as a Teacher’s Aide / Lunch person.
2. Laura Sternowski, Paraprofessional, School Department, purchasing first 6 months of employment with the school department as a Teacher’s Aide.
3. Danielle Payne, Principal Clerk Typist, School Department, purchasing 2 years of employment at the school department as a part time data office temp.

Agenda Item #7- Steven Frappier/Police Department
Vote to accept the election of 12(2)(d) benefits from Mrs. Nancy Frappier, surviving spouse of Steven Frappier.

Agenda Item #8-Accidental Disability-Alejandro Colon/D.P.W.
Vote to consider the responses of the medical panel regarding the application for Accidental Disability benefits submitted by Alejandro Colon.

Agenda Item #9- Accidental Disability-William McDonald-Facilities
Vote to consider the acceptance of the application for Accidental Disability benefits submitted by William McDonald, and request that PERAC convene a medical panel.
