Agenda Item #2-Superannuations
Vote to consider the acceptance of the following Superannuation applications submitted by:
1.Barbara Cummings-Paraprofessional/School Department with 28 years and 4 months as of 6/27/14
2.Suzanne Defranco-Union President/Paraprofessional with 20 years and 1 month as of 6/21/14
3.Eva Pizarro-Cost Control Clerk/Buildings Department with 34 years and 5 months as of 7/22/14
4.Carlos Santana-Police Officer for the Police Department with 32 years as of 7/7/14
5.James Tower-Groundskeeper for the SHA with 10 years and 1 month as of 6/14/14

Agenda Item #3-Warrants
Vote to consider the payment of the following warrants
# 20 Refunds in the amount of $121,058.60
# 21 May Retiree Payments in the amount of $5,467,656.24 including $4.6 million from the PRIT fund.
# 22 Expenses in the amount of $202,657.74

Agenda Item #4-Refunds
Vote to consider seventeen (17) requests for refunds. The names have been submitted to their particular Department, to the Law Department and to the Massachusetts Department of Revenue.

Agenda Item #5-Redeposit of refund
Vote to consider the request to redeposit the refund from:
•Michelle Brier to redeposit the refund from the Springfield Retirement System, total 1 year, 1 month
•Geneva Caffee to redeposit the refund from the Springfield Retirement System, total 9 years, 8 months

Agenda Item #6-Makeup of contributions
Vote to consider the request to makeup contributions from:
•Erica Decker for 8.5 months in the Parks Department

Agenda Item #7-Involuntary Accidental Disability-Juan Ocasio/Police
Tabled from previous meeting for additional medical information.

Agenda Item #8-Involuntary Accidental Disability-David J. Barton/Police Department
Vote to consider the accidental disability benefit along with the evidence of the medical panel and the
involuntary application submitted by Commissioner William Fitchet.

Agenda Item #9- Accidental Disability-Richard D. Brohman/Fire Department
Vote to consider the accidental disability benefit along with the evidence of the medical panel and the accidental disability application

Agenda Item #10 – Accidental Disability-Karl Johnson/Police Department
Vote to consider the accidental disability benefit along with the evidence of the medical panel and the accidental disability application.

Vote to consider the approval of 12-2-d benefits to the surviving spouse of Police Officer, Lee Prentice who passed away on, May 26th, 2014.
